According to Article 37 of Occupational Safety and Health Act, employers shall notify a labor inspection agency within eight hours of the occurrence of any one of the following types of occupational accidents (referring to illnesses, injuries, disabilities, or fatalities suffered by workers due to factors such as buildings, machinery, equipment, raw materials, chemicals, gases, steam, dust, work activities, or other occupational causes in the workplace) at a business entity’s place of duty:
1. Accidents involving death;
2. Accidents causing injuries to three people or more;
3. Accidents causing injuries to one or more persons that require hospitalization;
4. All other categories of accidents designated and officially announced by the central competent authority.
If any of the situations mentioned above occur involving faculty or staff, on-campus workers (including contractors), and Work-study Program students, the victims, unit administrator, colleagues, site responsible personnel, or contracting units should fill out the attachment in this e-mail: Notification Form for Major Occupational Accident Cases and notify Environmental Protection, Occupational Safety and Hygiene Center (EPOSH Center) as soon as possible.
